Our commitment to your privacy
NVOPS is committed to ensuring the privacy and confidentiality of all personal information and follows the terms and conditions of privacy and confidentiality in accordance with the Australian Privacy Principles (Commonwealth Privacy Act 1988). The Privacy Policy communicates how NVOPS collects and manages personal information.
Personal information collected about you
We collect and hold the following information:
your name, address, date of birth, email and contact details
information about family or friends for next of kin contact
information about other health professionals involved in your care
any government and insurance identifiers such as, NDIS participant number, DVA number. These numbers are not used these for the purposes of identifying you within our practice.
Invoicing details including details of third-party funding organisations
Health information including;
diagnoses
symptoms
medical history
your appointment and invoicing details
results of assessments, recommendations, orthoses/prostheses prescribed/provided
specialist reports
imaging reports
Other information related to providing your care.
How we collect and hold your personal information
Directly from you when you give us your details (e.g., face-to-face, over the phone, via registration form or an online form)
from a person responsible for you
from a third party where we are permitted by law to do that (eg. other health care professionals involved in your care, your NDIS support co-ordinator
Why we collect information about you
NVOPS needs to collect information about you for the purpose of providing orthotic/prosthetic services. Information is also required:
to manage our accounts and administrative services, including billing, arrangements with health funds, pursuing unpaid accounts, management of our IT systems
to conduct accreditation, quality assurance or internal audits.
When and why, we share information about you with others
We may disclose information about you to others outside of our practice as permitted or required under law. This will include situations where we disclose information about you in order:
to comply with our legal obligations (e.g. mandatory reporting under legislation, responding to a court order or subpoena)
in order to consult with your healthcare team NVOPS may disclose information related to your orthotic/prosthetic management
to get test results from diagnostic services
to claim on insurance and other third-party funders
to communicate with your health fund, with government and other regulatory bodies such as
to help us manage our accounts and administrative services
to lessen or prevent a serious threat to a client’s life, health or safety or a serious threat to public health or safety
to help in locating a missing person
to prepare the defence of legal proceedings
to discharge notification obligations to liability insurers
to assist with collection of feedback from clients or other parties
NVOPS will not disclose your information to commercial companies, however specific service or product information suitable for your management, may be forwarded to you by NVOPS.
Your right to seek access and correction of your information
NVOPS takes all reasonable steps to ensure that information collected about you is accurate, complete and up to date. You may have access to your information on request. Please contact the practice in writing to request your information. NVOPS aim to respond within 30 days.
We may charge a fee to cover administrative and printing costs if we provide you with a copy of information we hold about you.
If you think that the information we hold about you is incorrect, let us know in writing. From time to time, we may also ask you to verify that the information we hold about you is correct and current. And please notify us if and when your contact details change.
How we hold your information
NVOPS take reasonable steps to protect your information. These are designed to prevent unauthorised access, modification or disclosure and to prevent misuse and loss. Steps include:
external company employed to manage digital systems, including cybersecurity threats
information is held in secure cloud storage
regular review and updating of NVOPS Privacy Policy, including staff education
access to information is restricted within NVOPS
strong protections are required to access digital information

provide incomplete or inaccurate information
withhold personal health information or
are unwilling to sign our privacy agreement
NVOPS may not be able to provide services.
